Only one spouse needs to include all the children's information. Birthdates are never published but are very helpful in estimating School of Discipleship classes.
There are three means of joining Redeemer, please read the descriptions and then check the appropriate box below.
- I have not joined a church since receiving Christ and I am joining Redeemer by profession of faith in Christ. (This means that you became a Christian through some connection with Redeemer and/or this is your first time joining a church.)
- I am a member in good standing of another church and I wish to have Redeemer write my former church to request a letter of transfer.
- Neither of the first two statements would be true of me. Therefore, I am joining Redeemer by reaffirmation of faith. (This means that you have been a member of a church, and for whatever reason, are no longer in contact with them or on their membership rolls.)

For each of your baptized children, please give the date, church, city, and name of the person who performed their baptism, to the best of your knowledge.

Please provide a brief account of your conversion to faith and repentance in Jesus Christ, i.e., your testimony. Briefly explain how and when God graciously brought you to Himself. If raised in a Christian home, please describe that childhood experience as well. Thank you for writing your story – it will help the pastor to get to know you better.
